A Form can be made accessible at a web URL for public users, i.e where it can be filled in and submitted by non-Upvise users.

 What can I use Public forms for?

Public forms can be used for any basic data capture for user that do not have access to upvise.

Some scenarios are listed below:

  • Site inductions for Subcontractors/ site visitors/non-employees

  • Anonymous Questionnaires to collect feedback

  • Collecting Data on a job site- e.g. what material did you dump in a bin?

  • Hazard or Near miss reporting

  • Prequalification assessments for subcontractors

 How do I convert my public form into a QR code?

You can turn the URL for your public form into a QR code so you can print and maintain it on-site.

Most browsers, such as google chrome, have inbuild QR code generators, otherwise, there are a number of websites where you can generate QR codes for free.

An example of where a QR code may be useful is when you have a lot of people coming and going from a job site, and every time they enter they may need to complete a check-in form, print the QR code, stick it on the entry to the induction point of your job site (or with the site supervisor). Anyone with a mobile will be able to scan the code and fill in the form.

To enable a Form for public access, edit the Form Template in your web account, and open the 'Sharing' tab. Click on Enable Public to get the URL of the page where the Form can be accessed publicly.

N.B.: not all fields types are supported in Public forms.

 Fields in Public Forms

Supported Fields:

  • Section,

  • Combo Box except Upvise data lists,

  • Question,

  • Check Box,

  • Label,

  • Text,

  • LongText

Non-Supported Fields:

  • Photo, Button, Signature, Contact and other Upvise data Combo Boxes will not be displayed in Public Forms.

  • Javascript code in Default value, Onchange, Execute on Submit, is not supported.
